Our Commitment To Privacy
Your privacy is important to us. To better protect your privacy we provide this notice explaining our online information practices and the choices you can make about the way your information is collected and used. To make this notice easy to find, we make it available on our homepage and at every point where personally identifiable information may be requested.
The Information We Collect:
This notice applies to all information collected or submitted on the MyMedicalSummary website. On some pages, you can place orders, make requests, and request for more information. The types of personal information collected at these pages are:
Name
Address
Email address
Phone number
Medical summary information that you may choose to upload to Google Health
Credit/Debit Card Information (Google Checkout)
We use the information you provide about yourself when placing an order only to complete that order. We do not share this information with outside parties except to the extent necessary to complete that order.
We use the information you provide about someone else when placing an order only to ship the product and to confirm delivery. We do not share this information with outside parties except to the extent necessary to complete that order.
The medical records that are sent to MyMedicalSummary will be used for processing. If you send paper medical records to MyMedicalSummary, we will create digital images of those paper medical records, and ask you to indicate whether we should shred those paper records upon completion of processing, or return them to you (return shipping fees apply). MyMedicalSummary will not disclose, or create physical copies of your medical records. We encourage individuals not to send the original copies of their paper medical records to MyMedicalSummary during this process.
MyMedicalSummary will extract medical information, such as conditions, medications, allergies, surgeries, procedures, and immunizations, from the digital images of your medical records, and create an organized summary for you. The summary information and organized digital images will be mailed back to you on a compact disc.
You may elect to upload the summary information into your Google Health profile, and if you do, a copy of your MyMedicalSummary will also be stored on our secure servers. At your direction, we will upload all or portions of your MyMedicalSummary to your Google Health profile. After your summary information is uploaded to Google Health, you may elect to delete or to maintain for future use the summary information stored on our MyMedicalSummary secure server.
We do not aggregate or summarize the health information stored in medical summaries by our users. We do not sell nor share nor provide health information, individually nor in aggregate, to any third party or individual.
Finally, we never use or share the personally identifiable information provided to us online in ways unrelated to the ones described above without also providing you an opportunity to opt-out or otherwise prohibit such unrelated uses.

To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, we have put in place appropriate physical, electronic, and managerial procedures to safeguard and secure the information we collect online.
Our Commitment To Children's Privacy:
Protecting the privacy of the very young is especially important. For that reason, we never collect or maintain information at our website from those we actually know are under 13, and no part of our website is structured to attract anyone under 13. If you are a parent guardian acting on behalf of a child under the age of 13, we request that you contact us at info@mymedicalsummary.com.
How You Can Access Or Correct Your Information
If you submitted an order through MyMedicalSummary you will receive an invoice. If for any reason you have provided MyMedicalSummary with incorrect information during the checkout process then please respond to the invoice including the corrected information immediately.
